Sources say the vehicle tipped over and that Ms O’Keane suffered a severe brain injury. She was taken to the Mater Misericordiae University Hospital, placed on a ventilator in the ICU and died of her injuries on Thursday, 1 October. No other injuries were reported at the scene.
Gardaí request footage and information
The Gardaí have asked anyone who was travelling in the North Wall Quay area between 10.30pm and 11.15pm on Saturday, 26 September 2026 to check for camera or dash-cam footage and to make it available to investigators.

Anyone with information is asked to contact Store Street Garda Station on 01 666 8000, the Garda Confidential Line on 1800 666 111, or any Garda station.
Family reaction and tributes
Ms O’Keane, a 37-year-old from Newry and mother of two, has been remembered by friends and relatives for her kindness and devotion to her family. Messages posted online described her as “the most beautiful girl and devoted mum,” while others paid tribute to her warmth and work colleagues said they were thinking of her family.
It is understood that Ms O’Keane was struck by a car after falling from the rickshaw. Her sister-in-law, Aisling, told Newstalk the evening had been intended as a celebration and described the scene after the gig as chaotic. “It’s like Final Destination,” she said, recalling how the rickshaw clipped a divider, tipped over and was then involved in a collision.
Ongoing inquiries
Investigations remain ongoing as Gardaí examine evidence and gather witness accounts. The force has reiterated the importance of any video or photographic material from the time and location to assist with establishing the full circumstances of the incident.
